Not all white lumps in the tonsil are tonsil stones.

This photo shows a submucosal white patch on the left, it has tiny blood vessels over it surface so it is therefore under the surface. The white patch on the right has no vessels, it is a tonsil stone and you can see a deep hole in the tonsil next to it, where it is originating from.
